Instructions:  Return completed form to Northside Education Center, 707 Fahrenthold Street, El Campo, Texas 77437.  Reservation is not confirmed until it has been returned to you with approval.  Allow approximately seven days from receipt of request for the Northside Center to review your request.  Facility rates and guidelines may be found in the Facilities and Grounds Usage Policy.  The Form is available from Northside Education Center.
